A Case of Limbic Encephalitis Associated with Bilateral Ovarian Teratoma

Abstract
A specific type of paraneoplastic limbic encephalitis (PLE) has been associated with ovarian teratomas. Twenty-year-old
female patient visited our clinic presenting with acute delirium and intermittent generalized tonic-clonic seizure for 1 day. The initial brain MRI showed the leptomeningeal enhancement without abnormality of brain parenchyme. CFS studies revealed pleocytosis (WBC: 18/mm2) while level of glucose and protein (82 mg/dL, 33.3 mg/dL, respectively) were within normal range. With the consideration of viral encephalitis, antiviral and anti-epileptic agent started to infuse intravenously. However, refractory seizure and confused mentality did not improve. On the further work-up, bilateral mature teratoma was found out at abdominal CT. After ovarian cystectomy, mental status recovered to alert and neurological symptoms such as seizure were improved. We reported with an young age patient of PLE with ovarian teratoma, which has been rarely reported.
